123music periodically conducts online surveys on the Site, which help us to assess the interests and objectives of the musicians and citizens who use our Site. While our surveys may request demographic data such as age, gender, and household income to help us better evaluate the results, we do not correlate this demographic information with the identity of the individual who provided it. In some cases, we may give survey participants the option of requesting an emailed copy of our final survey results. The email address submitted by the survey participant for this purpose is not correlated with his or her survey responses. Survey participants are not required to provide an email address in order to participate in our surveys.
Our Site facilitates individual participation in policy-based actions, such as the submission of public comments to the FCC or other government entities. At times, the Site may provide a mechanism allowing individuals to submit public comments to the FCC by using a comment form provided on the Site. The form identifies the topics on which public comments are requested and is intended to help participants craft their responses. Submitting public comments to the FCC requires the participant to provide his or her name, address, and email address. Public comments submitted through our Site are automatically transferred to the FCC. 123music does not maintain copies of the comments submitted to the FCC through our Site, and we do not use contact information provided in public comments submitted through our Site to communicate with those individuals.
123music sponsors the Health Information Navigation Tool (“HINT”), which helps to provide musicians with basic information about obtaining health insurance coverage. Our Site allows you to schedule a telephone appointment with a HINT representative. In scheduling your telephone appointment through the Site, you will be asked to provide your first name; your city, state, and zip code of residence; the telephone number to be used to reach you for this appointment, and your email address. You may also elect to provide other types of personal information through the Site, such as your age, gender, marital status, number of children, and information about your income and current insurance coverage. This information helps the HINT representative begin to identify types of coverage that may be available to you, and helps us to understand the needs of HINT users. Please refer to our Terms of Service for other important information about HINT.
Our Site is not directed to children and we do not knowingly collect personal information from individuals under eighteen years of age.
